Traveling Soon? VA Offers Seamless Care for Traveling Veterans or Veterans Seeking Care at an Alternate VA
Veterans who spend time living in more than one place during the year need to know how to receive care from more than one VA facility. We want you to understand how to get VA health care while away from your “home” facility.
Once you know you will be traveling, notify your local VA Patient Aligned Care Team (PACT), preferably (4) four to (6) six weeks prior to departure, or as soon as you are aware of your travel plans. Early planning will allow time for your local PACT and the Traveling Veteran Coordinator to coordinate your care at an alternate VA health care facility.
Before You Travel
The key member of the health care team is the patient. As a team member, there are some actions a patient may take to make timely healthcare, and travel, a good combination. Prior to leaving home the Veteran should:
- Touch base with your Primary Care Team - You’ll need to let your PACT know travel destination(s) , mailing address while you are on travel (for medication delivery), telephone number, any specific care concerns and your arrival and departure dates.
Your PACT will contact the Traveling Veteran Coordinator, who will assist in coordinating your care at the alternate VA health care facility for any care required during your travel dates for any ongoing medical care needed. All required specialty care should be coordinated through the “home” facility traveling Veteran coordinator.
