Bay Pines VA Healthcare System is happy to offer an Advanced Care Planning Workshop  held at the Sarasota Community Based Outpatient Clinic from 1 – 2:30 p.m. Due to limitations on space, registration is required.

These workshops are open to all Veterans and a guest of their choice.

Advance Care Planning Group Visits (ACP-GV) offer a place for you to share, learn, and help your fellow Veterans understand the benefits of planning for future health care decisions if you become too ill to make or communicate choices for yourself. ACP-GV can help you understand how to fill out an advance directive that matches your preferences, so you are always in control of your health care.
