This document is not issued by CMS and does not represent an official CLIA fee or government notice. Official CLIA communications come only from CMS or your State Survey Agency.

Get the Same Information for Free – CMS already provides this compliance and survey information to the public at no charge through the Quality, Certification and Oversight Reports (QCOR) system. Use the QCOR CLIA Laboratory Lookup to view your laboratory's certification status directly.
Reminder: CLIA Is Paperless – CMS no longer mails paper CLIA certificates or fee coupons. All certification and survey fees must be paid online, and paper checks are no longer accepted. Keep a valid email address on file to receive your electronic certificates, fee coupons, and other CMS communications.
