When 21-year-old Dale Alice Martin was admitted to Connecticut’s Waterbury Hospital for a broken ankle, her parents had no real cause for concern. The next day they received a frantic call saying Dale had been found unresponsive and was now in intensive care. Tests confirmed that she had no brain activity.

The medical examiner later ruled that Dale had died due to an accidental over-administration of narcotics. “There was no need for her to die,” Dale’s father, Lindsay Martin, said.

Dale’s parents had no idea that she ended up at a medical facility in crisis.
