CORE Outbreak Investigation Table
The CORE Outbreak Investigation Table is a list of outbreak and adverse event investigations primarily being managed by FDA’s CORE Response Teams. The investigations are in a variety of stages, meaning that some outbreaks have limited information, and others may be near completion. The table will be updated weekly.
This week's updates are:
- A new outbreak of E. coli O157:H7 (ref #1421) has been added to the table. USDA’s Food Safety and Inspection Service (USDA-FSIS) reference number (2026-04) for the investigation has been added to the table. FDA has initiated traceback.
- A new outbreak of Listeria monocytogenes (ref #1417) has been added to the table. FDA has initiated traceback.
- For the outbreak of Salmonella Newport (ref #1403), the case count has increased from 111 to 116. FDA has initiated sampling.
- For the outbreak of E. coli O145:H28 (ref #1389) linked to frozen blueberries, CDC has declared that the outbreak has ended. FDA updated the outbreak advisory on September 21, 2026, to provide final investigation updates. FDA’s outbreak investigation has closed.
- The outbreak of Cyclospora (ref #1398) has ended. FDA’s outbreak investigation is active.
- The outbreak of Cyclospora (ref #1392) has ended. FDA’s outbreak investigation is active.
- The outbreak of Cyclospora (ref #1385) has ended. FDA’s outbreak investigation is active.
- For the outbreak of Salmonella Typhimurium, Newport, and Richmond (ref #1358) linked to Moringa Leaf Powder, FDA’s outbreak investigation has closed.
